2 Clinical Rotations in 1 Month – Medical Student Rotation
What Is This?

What Is a Combination Rotation for IMGs?

A combination rotation (combo rotation) is a structured USCE program where an IMG completes rotations at one / two different US teaching hospitals within a single month β€” earning two separate faculty LORs and maximising clinical exposure without extra time or cost and saving you money.

Each rotation is for 2 weeks, but the rotations are for longer hours; you will get credit for the whole month. The combination rotation will start every month. You will receive one letter dated for the current month and another will be in the following month.

For IMGs applying to the NRMP Main Residency Match, US Clinical Experience is one of the most important differentiating factors. Program Directors consistently favour applicants who have meaningful, hands-on, inpatient experience at accredited teaching hospitals with strong LORs from physicians holding official faculty designations.

At Medical Student Rotation, all combination packages are 100% inpatient and hands-on, at hospitals with active residency programs, supervised by Chairmen, Chiefs, or Heads of Department.

Common Questions

Frequently Asked Questions

Everything IMGs typically want to know before enrolling in a combination rotation in the USA.

What exactly is a combination clinical rotation for IMGs? +
A combination rotation (or combo rotation) is a USCE program where you complete hands-on inpatient training at one / two different US teaching hospitals within a single 4-week period. You receive two separate Letters of Recommendation from one / two different attending physicians β€” doubling your LOR count without doubling your time or cost.
Why are combination rotations better than two separate single rotations? +
Two separate back-to-back rotations mean two months of rent, food, transportation, and time away from home. A combination rotation delivers the same outcome β€” two strong inpatient LORs β€” in one month. Most students save between $1,500 and $2,000 by choosing a combo plan.
Are these LORs accepted by US residency programs? +
Yes. All LORs are written on official hospital letterhead by attending physicians holding faculty designations at their respective teaching institutions (Rosalind Franklin University or University of Chicago AdventHealth). LORs are individually written based on your performance, sincerity, and attendance β€” not generic templates.
Do both hospitals in the combo plans have residency programs? +
Yes. All hospitals and their affiliates in our plans have active residency programs. Program Directors view LORs from hospitals with residency programs as more credible, since the attending physicians regularly evaluate residents.
Can I get an LOR in a specialty other than Internal Medicine? +
Yes, your LOR can be issued in Internal Medicine, Neurology, or Nephrology β€” letting you tailor your application to your target specialty.
What documents do I need to start the rotation? +
You will need: MMR vaccination records, a PPD test (within 12 months), COVID-19 vaccination proof, full immunisation records, and your resume/CV. All documents should be emailed to info@medicalstudentrotation.com. Business casual attire, scrubs, or a lab coat are required for clinical days.
What is the accommodation situation for combination rotations in Chicago? +
A rotation fee + accommodation option is available. Accommodation is in a fully furnished house, within 5 minutes walking distance.
When can I start? Are there set start dates? +
Combination rotations are available on a rolling basis throughout the year. You can start the rotation on the 7th, 14th, 15th, or 21st of the month. Contact MSR on WhatsApp at +1-312-437-7866 for current availability or any questions.
What is the refund policy if I need to cancel? +
Full payment is due 8 weeks before the rotation start date. A 50% refund is issued if you cancel more than 4 weeks before the start date. No refund is available for cancellations within 4 weeks of the start date.
Is a combination rotation right for me if I'm an FMG with older USMLE scores? +
A combination rotation is especially valuable for FMGs with older exam scores or a gap in clinical activity. Two recent hands-on inpatient LORs from US teaching hospitals with residency programs demonstrate continued clinical engagement and directly address concerns Program Directors may have about applicants who graduated several years ago.
